<p><em>Background</em><br />
The purpose of the present study was to determine the airway changes in skeletal class II division 1 malocclusion patients with mandibular retrognathism, treated with Twin-Block (TB) appliance.</p>
<p><em>Methods</em><br />
Airway assessment was carried for twelve patients (mean age 11.7 ± 1.1 years) who underwent myofunctional therapy using TB appliance for correction of skeletal class II division 1 malocclusion with mandibular retrognathism. Acoustic pharyngometry (AP) was used to assess and quantify the comparative changes in the upper airway, pretreatment and posttreatment.</p>
<p><em>Results</em><br />
Data acquired was subjected to appropriate statistical analysis. The paired ‘t’ test was used to compare pre-treatment (T0) and after the positive pterygoid response (T1). TB appliance increased mean minimum airway area by 0.28 ± 0.25 cm2 and mean airway by 0.47 ± 0.44 cm2 with 95% CI. Posttreatment minimum airway and mean area changes were found to be statistically significant (P-value&lt;0.01).</p>
<p><em>Conclusion</em><br />
TB appliance therapy has a positive effect on upper airway and is beneficial for the treatment of sleep-related disorders associated with Class II division 1 malocclusion for achieving positive functional changes, esthetics, and healthier quality of life.</p>

<p><em>Background:</em> There is lack of reliable and accurate methods to predict treatment out- comes of oral appliance (OA) treatment. Acoustic pharyngometry (AP) is a non-inva- sive technique to evaluate the volume and minimal cross-sectional area of the upper airway, which may prove useful to locate the optimal position of OAs.</p>
<p><em>Objective:</em> This retrospective study aimed to evaluate the effect of applying AP to OA treatment of patients with obstructive sleep apnoea (OSA).<br />
Methods: All patients (n = 244) treated with OAs following an AP protocol at two dental clinics between 2013 and 2018 were invited to participate. A total of 129 pa- tients accepted the invitation, and 120 patients (75 men, 45 women) were included in the analyses. Mean baseline age, BMI and apnoea hypopnea index (AHI) were 59.1 ± 0.9 years, 27.8 ± 0.4 and 21.9 ± 1.1, respectively. Mean follow-up time was 318 ± 24 days.</p>
<p><em>Results:</em> AHI at follow-up was 6.4 ± 0.7, resulting in a treatment success rate of 86.7% (≥50% reduction of baseline AHI). The number of failures (&lt;50% reduction of baseline AHI) did not differ significantly among patients with mild, moderate and severe OSA. 87.6% of the patients reported OA usage every night, and 95.5% reported &gt; 5 hours usage per night, when worn.</p>
<p><em>Conclusion:</em> The AP protocol applied seems to contribute to the excellent effect of OA treatment in this study. Further research on the application of AP in OA treat- ment is necessary in order to clarify its possible beneficial contribution to improving OA therapy.</p>

										<content:encoded><![CDATA[<h3 class="deck">The ADA says dentists should be screening their patients for sleep apnea and should be the professionals who make oral appliances.</h3>
<p>In October 2017, the American Dental Association (ADA) adopted a policy on dentistry’s role in treating sleep-breathing disorders including obstructive sleep apnea (OSA). It emphasizes that all dentists should screen for sleep-breathing disorders and that dentists should be the professionals who fabricate oral appliances (with a physician’s prescription).</p>
<p>Sleep-breathing disorders are “recognized as potentially serious medical conditions caused by anatomical airway collapse and altered respiratory control mechanisms,” the ADA said in a written statement to Sleep Review. “As experts in the oral cavity, dentists are able to question patients about pertinent symptoms during routine dental evaluations, and refer patients to physicians for diagnosis. The policy was adopted to address dentistry’s growing role in the multidisciplinary care of patients with [sleep-breathing disorders].”</p>
<p>In a news release from October, the ADA lists key takeaways that describe the role dentists should play:</p>
<ul>
<li>assess patient risk for sleep-breathing disorders as part of a comprehensive medical and dental history and refer affected patients to appropriate physicians</li>
<li>evaluate the use of oral appliance therapy and provide it for mild and moderate OSA when CPAP fails</li>
<li>identify and address the side effects of oral appliance therapy</li>
<li>communicate patients’ treatment progress with referring physician and other healthcare providers.</li>
</ul>
<p>Another point made in the policy is dentists treating sleep-related breathing disorders should stay up-to-date on the disorders and training in dental sleep medicine with continuing education. When asked what education the ADA will be providing dentists about how to screen for sleep-breathing disorders, the ADA advised that is currently under consideration.</p>
<p>The policy, written by the ADA Council on Dental Practice, is a result of an evidence brief on the use of oral appliances for sleep-breathing disorders, which was developed by the ADA Council on Scientific Affairs. The ADA’s governing body, the House of Delegates, reviewed and made comments for the policy, which were incorporated into the final draft before the association finally adopted it this past fall.</p>
<p>The evidence brief that inspired the policy shows there is a large portion of people who could have undiagnosed sleep-breathing disorders that could potentially be identified and treated by dentists who abide by these policy guidelines. It states that the prevalence of OSA is “estimated to be 3% to 7% in men and 2% to 5% in women. Prevalence is higher (greater than 50%) in patients with cardiac or metabolic disorders, relative to the general population.”</p>
<p>The ADA does not yet have data on any recent increase in sleep-disordered breathing screenings at dental offices or how quickly the policy is being rolled out.</p>

Further pursuing his education in 1991, Dr. Doucette ventured north to Reno where he attended the University of Nevada, Reno and received his bachelor’s degree of science in 1995. He then attended Oregon Health Sciences University in Portland, Oregon where he received his Doctor of Dental Medicine in 1999.</div>

			<p>After his first few years in practice Dr. Doucette developed a passion for cosmetic dentistry and began to focus on smile revitalization through the use of technologically advanced surgical/ restorative cosmetic dental procedure. In 2009 after surgically placing dental implants for nearly ten years, Dr. Doucette successfully expanded his focus to the planning and placement of fixed- full- arch -implant- supported- prosthetics via CT-Guided technology when multiple implants were indicated to restore a smile.</p>
<p>As a result of Dr. Doucette’s extensive training over the last eighteen years, he has become acutely aware of the vital role that efficient sleep plays in the optimal health and healing of his patients so in recent years Dr. Doucette has become very passionate about heavily screening and treating all of his patients for sleep breathing disorders such as snoring and obstructive sleep apnea. Dr. Doucette continues to pursue excellence while remaining on the cutting-edge of dental and sleep health technology.</p>
